Effective Jan. 1, 2019, hospitals are required to have a homeless
patient discharge planning policy to prepare homeless patients
for return to their community by connecting them with:
available community resources
The Hospital Council convened members to identify effective
discharge planning strategies and additional approaches hospitals
and community partners could develop to help individuals who are
homeless in our community. Through this process, the Hospital
Council helped members establish relationships with government
and community partners, and gathered information to assist
hospitals with the new requirements.
