The Hospital Quality Institute (HQI) was established in April 2013 to realize statewide impact of improving patient safety and quality care for all Californians, to accelerate the rate of improvement, and to advance California as a national leader in quality performance.

Created in 2008 by the California Hospital Association, CHPSO is a federally designated Patient Safety Organization (PSO) dedicated to the elimination of preventable patient harm and improving the quality of health care delivery. With over 400 members in the United States, CHPSO is one of the largest PSOs in the nation and is a trusted leader in the analysis, dissemination and archiving of patient safety information. CHPSO collects safety event reports and other reports of patient safety issues in a legally protected manner.
HealthCare Reliability Organizing™ (HCRO) is a distinctive approach to achieving reliability in healthcare.The HCRO model includes transforming both individual learners and healthcare organizations through a multi-step process of continuous improvement. Rooted in the foundations of socio-technical science and developed in collaboration with world-leading reliability experts and teams of Cohort Organizations across California, HCRO is demonstrating that the path to sustainable healthcare reliability can be achieved.
The HSAG/HQI Partnership invites hospitals to join the HSAG Hospital Improvement Innovation Network (HIIN) and to be a part of the greater achievements going forward.
A continuation of our CalHEN work, the HSAG HIIN will support hospitals in sustaining and advancing improvement. Free of charge, programming will be customized to fit the unique needs and contexts of each hospital. Through a robust partnership of hospitals, enhanced by the expertise of leading national quality and patient safety experts, we will expand the use of best practices and accelerate improvement in critical areas of healthcare. Over 270 hospitals have already expressed interest to commit to the HSAG HIIN and we’d like to include you!
Focusing on patient experience is key to creating a high-quality health care system. P4PEx is our vehicle for assisting California hospitals with systemic advances in patient experience and engagement. Emphasis on cultural competence is a natural complement given California’s demographic diversity.
